Description

Immunisation is a simple, safe and effective way of protecting your child against certain diseases. The risks from having these diseases are far greater than the risk of any minor side effects from immunisation.

A full schedule of the recommended childhood immunisations are listed below.

At any time you can download a 'Vaccine Record Request Form' from our Useful Forms section on the website and hand it into reception at the surgery. The Practice Nurse will then print out the vaccination record and this will be available 48 hrs later for collection from the surgery. Please note that we will only have a record of the vaccines administered at Elmwood.
